PRACTICE POLICIES

New patients

Please arrive 45 minutes before your scheduled time. We highly recommend you complete your paperwork before you arrive. The paper work is available on the website under the For Patients/forms tab . You can choose to fax or e-mail  it to us in order to save time or bring it with you. Patients need to bring in all their pill bottles for prescribed and over the counter medications they take and any old records.

Established patients

Please arrive 15 minutes before to verify your insurance and update your demographic information and update your health history. Patients who take more than (3) medications please bring all your medications in their bottles  at all your office visits.

Refill request and phone messages

We recommend you call your pharmacy for refill requests and give us 48 hours for response. Due to the high call volume we suggest you use the patient portal to communicate with us. All urgent phone and portal messages will be returned on the same day and non-urgent messages will be returned within 24 hours.

Late arrivals

If you arrive later than 15 minutes after your scheduled appointment time your appointment may be rescheduled depending on how busy the schedule is for that day.

Cash pay patients

We pride ourselves in offering a competitive fee for service for those patients who do not carry any insurance. Download the self-pay fee schedule PDF.

No show fees

We require a 24 hour advance notice of appointment cancellation. A $25 fee will be applied to your account for short notice cancellations and no shows. Patients who repeatedly No Show may be discharged from the practice.

Annual exams and well women exams

An annual exam is required yearly for all the patients of our practice. During this visit we review all your medical and family history including updating your medications and recommend the appropriate cancer screening.

Emergencies/After hours

To provide the best service to our patients one of our doctors is on call 24 hours a day to avoid you going to the Urgent Care. If you would like to reach the doctor on call after hours, please call our office number and follow the phone prompts. If you feel it is an emergency matter call 911.

Payment

All co-pays, deductibles and account balances are DUE at the time of the visit.
